• Med-Connect (Medicaid for Employees with Disabilities)

  Medicaid for Employees with Disabilities, also known as MED-Connect, provides medical assistance to employed individuals with disabilities. Enrollees can earn up to $75,000 per year and qualify for full Medicaid/HUSKY Health coverage.

• Long-Term Services and Supports - LTSS

  Governor Dannel P. Malloy, joined by Department of Social Services Commissioner Roderick L. Bremby and Office of Policy and Management Undersecretary Anne Foley, announced the release of Connecticut’s Strategic Plan to Rebalance Long-Term Services and Supports, including a town-by-town projection of Connecticut’s long-term care needs and strategies to meet those needs, on January 29, 2013.

• Medicare Savings Program

  The State of Connecticut offers financial assistance to eligible Medicare enrollees through our 'Medicare Savings Programs.' These programs may help pay Medicare Part B premiums, deductibles and co-insurance.

• Money Follows the Person Program

  Money Follows the Person is a Federal demonstration project dedicated to assuring Connecticut residents access to a full range of high quality, long-term care options that maximize autonomy, choice and dignity.

• Long-Term Care Ombudsman Program

  The Long-Term Care Ombudsman Program protects the health, safety, well-being, and rights of individuals who reside in skilled nursing facilities, residential care homes and assisted living facilities.

• Aging and Disability Resources Center

  Aging and Disability Resources Centers provide consumers of any age and ability with benefits counseling, person-centered planning to help identify available long and short term services and supports, application assistance and general information and assistance.

• Elderly Nutrition Program

  The purpose of the Elderly Nutrition Program is to provide meals, reduce hunger and food insecurity, promote socialization and the health and well-being of older adults. The program provides nutritionally balanced meals to eligible residents 60 years of age and over and their spouses. Meals are served through community cafes or delivered to the home-bound older adults.

• The Congregate Housing Services Program (CHSP)

  The Congregate Housing Services Program (CHSP) provides supportive services for participants in eligible housing projects who meet specific requirements. The supportive services may be provided to frail, elderly persons at least 62 years of age or non-elderly persons with disabilities for the purpose of promoting independence, preventing unnecessary institutionalization and helping people remain in the community as long as possible.

• The CHOICES Program

  Connecticut's program for Health insurance assistance, Outreach, Information and referral, Counseling, Eligibility Screening. This program provides information to persons age 60 and older as well as to people with disabilities, and is comprised of both staff, in-kind professionals from local service agencies and volunteers.

  The Birth to Three System strengthens the capacity of families to meet the developmental and health-related needs of their infants and toddlers who have delays or disabilities. Citizens can find information on the various services the Birth to Three System offers.

  The Division of Family Support Services provides a comprehensive approach to preventing child abuse and neglect to ensure the health, well-being and positive growth and development of all young children in Connecticut.

  The Division of Licensing is responsible for the administration of child day care and youth camp licensing programs at the Office of Early Childhood. Family day care homes, group day care homes, child day care centers and youth camps are required to be licensed.
